Can Fenugreek Lower Blood Sugar in Type 2 Diabetes?
Several reviews of trials found fenugreek seeds lower HbA1c, a long-term blood sugar test, in type 2 diabetes. The effect is modest and the trials are small.

Short answer
Fenugreek seeds modestly lower long-term blood sugar (HbA1c) in type 2 diabetes. The evidence is limited, so it is not a substitute for diabetes medicine. Evidence grade C (Limited). Meta-analyses of randomised trials agree on a modest HbA1c fall; downgraded for inconsistency between trials and because HbA1c is a blood marker rather than a diabetes complication.
Key takeaways
- Pooled trials show fenugreek lowers HbA1c, a standard long-term blood sugar test, by about 0.5 to 0.9 percentage points [1][2][3].
- Fasting blood sugar results are mixed: one review found a fall, another found no significant change [3][2].
- Most trials are small and used different forms and doses, so results may not carry over to other fenugreek products [4][1].
- A 2023 meta-analysis found no severe side effects in the trials, but a 2025 case report links a fenugreek supplement to liver injury in a woman also taking a cancer drug [1][5].
The short version
Fenugreek seeds appear to lower HbA1c, a long-term blood sugar test, in people with type 2 diabetes. The effect is modest, and the trials are small and varied, so the evidence is limited [1][2].
What the studies show
A 2023 meta-analysis of randomised trials in 706 people found that fenugreek lowered HbA1c by 0.54 percentage points on average, with a wide range between trials [1]. A 2023 review of 14 trials, with 894 people, reported a fall of 0.88 points [2]. An earlier 2014 review found a fall of 0.85 points, based on three trials that measured it [3].
Fasting blood sugar results are less consistent. The 2014 review found fasting glucose fell by 0.96 mmol/L, but with large differences between trials [3]. The 2023 review of 14 trials found no significant change in fasting blood sugar [2]. A separate 2023 review of 10 trials, with 706 people, did find significant falls in fasting and two-hour blood sugar [1].
Individual trials point the same way but are small. In an 8-week trial of 88 adults with type 2 diabetes, 10 g of powdered whole seeds a day lowered HbA1c and fasting blood sugar more than wheat starch placebo [4]. A 14-day crossover study of 21 adults with type 2 diabetes found that 30 g a day of defatted seed flakes (FenuflakesTM) lowered 24-hour blood sugar compared with a control diet; 15 completed the trial [6].
The nuances
Doses and forms differ. Trials used from 2 g a day [7] to 25 g twice a day for a month [8]. In the 2023 meta-analysis, whole-seed powder lowered HbA1c more than standardised extracts in subgroup analysis (-0.95 versus -0.34 points) [1]. Results for one product do not carry over to another.
Some trials did not show a clear benefit. In a 12-week trial of 12 patients on metformin, 2 g a day of fenugreek did not significantly lower fasting glucose compared with glibenclamide, and fasting insulin rose; nine patients finished [7]. A seven-herb Iranian combination that included fenugreek lowered fasting glucose and HbA1c versus placebo, but the trial cannot show how much fenugreek contributed [9].
The 2014 review found the fasting and two-hour glucose benefits were mostly seen in people with diabetes at medium to high doses [3]. A 2026 crossover pilot in 18 healthy young adults found that a flatbread with 7.5% sprouted fenugreek reduced after-meal blood sugar and insulin compared with white flour, but healthy people are not the target group for diabetes control [10].
The 41038677 trial was funded by a company that makes the product, and its authors are employed by the sponsor [6].
What this means for you
If you have type 2 diabetes, the studies do not suggest fenugreek replaces diabetes medicine. Several trials gave it alongside usual treatment. Fenugreek may add to the effect of blood sugar medicines, which can cause the blood sugar to drop too low [11]. Talk with your doctor or pharmacist before using fenugreek supplements, especially if you take diabetes medicine.
Safety
In the 2023 meta-analysis, the trials reported no hepatic or renal toxicity and no severe adverse events, with mild gastrointestinal side effects in some [1]. A 2025 case report describes a 47-year-old woman with liver injury from the cancer drug ribociclib, whose liver enzymes returned to normal after she stopped both ribociclib and a fenugreek supplement; the authors judged a possible herb-drug interaction [5]. No trial of fenugreek in pregnancy was found. A 2020 Cochrane review of oral galactagogues, which are used to increase breast milk, found very uncertain evidence and only minor side effects reported for fenugreek [12]. No data were found for people with kidney disease or bleeding disorders.
