Fenugreek
Last reviewed October 2026 · Based on 16 studies
Bottom line
Fenugreek seeds modestly lower HbA1c, a long-term blood sugar test, in people with type 2 diabetes, according to several meta-analyses. Fasting blood sugar results are mixed and most trials are small. Safety data are limited: one case report links a fenugreek supplement to liver injury alongside a cancer drug.
What it helps with
- Lowers long-term blood sugar (HbA1c) — Benefit, evidence grade C (Limited). Several meta-analyses of RCTs report HbA1c falls of 0.54 to 0.88 points; downgraded for inconsistency between trials and because HbA1c is a blood marker, not a diabetes complication.
- Lowers fasting blood sugar — Mixed results, evidence grade C (Limited). Reviews disagree: one found a fall of 0.96 mmol/L, another no significant change; downgraded for inconsistency with high heterogeneity (I2 80%).
What it is
Fenugreek (Trigonella foenum-graecum) is an annual plant whose seeds are used in cooking and in traditional medicine for diabetes [1]. Studies use whole-seed powder, standardised extracts and defatted seed flakes [1][2].
What the research says
Meta-analyses of randomised trials show that fenugreek lowers HbA1c by about 0.54 to 0.88 percentage points [1][3][4]. Fasting blood sugar results are mixed: one review found a fall of 0.96 mmol/L [4], while another found no significant change [3]. Trials are small and vary in form and dose, so the effect size is uncertain [1].
Doses used in studies
Trials used doses from 2 g a day [5] to 10 g a day for eight weeks [6], and 25 g twice a day for one month [7]. These are study doses, not recommendations.
Safety and interactions
Side effects: mild gastrointestinal complaints were reported in some trials, and no severe adverse events were found in the 2023 meta-analysis [1]. Medicine interactions: fenugreek may add to the effect of diabetes medicines and cause low blood sugar [8]; a case report describes a probable herb-drug interaction with ribociclib and liver injury [9]. Pregnancy and breastfeeding: no trial of fenugreek in pregnancy was found; galactagogue evidence is very uncertain [10]. Upper limits and toxic doses: none were found in the trials reviewed. Kidney disease and bleeding disorders: no data were found. People with diabetes on medicines should talk to a doctor first.
Caveats and open questions
Fasting blood sugar results conflict between reviews [4][3]. Several trials are small or low quality, and one trial of 12 patients did not show a fasting glucose benefit against glibenclamide [5]. One crossover trial was funded by the company that sells its product [2]. Whether whole seed, extract or flakes works best is not settled [1].
